Figueroa Sanchez, Arch DBA Figueroa & Figueroa: The subcontractor will provide Architectural and Engineering Services for the construction of the Municipal Gym of Sabana Grande Municipality under the City Revitalization Program of CDBG-DR Funds. The contract term will be from April 17, 2023, until March 31, 2024. The contract amount will not exceed $116,000.00. UEID: UCNFZ176DYF4.
Figueroa & Figueroa: Amendment A- The subcontractor will provide Architectural and Engineering Services for the construction of the Municipal Gym of Sabana Grande Municipality under the City Revitalization Program of CDBG-DR Funds. Amendment A corrects the item number to read “PARTIDA NUMERO: 70-13-04-9238 ID: CITY REV.” All other terms and clauses including the contract amount will remain unaltered. UEID: UCNFZ176DYF4.
Figueroa & Figueroa: Amendment B- The subcontractor will provide Architectural and Engineering Services for the construction of the Municipal Gym of Sabana Grande Municipality under the City Revitalization Program of CDBG-DR Funds. Figueroa & Figueroa: Amendment C- The subcontractor will provide Architectural and Engineering Services for the construction of the Municipal Gym of Sabana Grande Municipality under the City Revitalization Program of CDBG-DR Funds. Amendment C corrects the item number to read “Partida Presupuestaria 70-13-04-94.11 ID: CITY REV.” and extends the contract term from March 21, 2024 until June 30, 2024. Figueroa & Figueroa: Amendment D- The subcontractor will provide Architectural and Engineering Services for the construction of the Municipal Gym of Sabana Grande Municipality under the City Revitalization Program of CDBG-DR Funds. Amendment D adds clause “SEXTAGESIMO: FEDERAL AND HUD CLAUSES REQUIRED” All other terms and clauses, including the contract amount, will remain unaltered. UEID: UCNFZ176DYF4.
CVS CONSULTANT AND PROJECT MANAGEMENT, P.S.C.: (2023-000072) The subcontractor will provide professional services for the Design, Measurement, Soil Study, Engineering and Supervision for the Luis Muñoz Rivera School of Artistic and Cultural Development under the City Revitalization Program of CDBG-DR Funds. The contract term will be from August 15, 2022, until July 7, 2023. The contract amount will not exceed $99,450.00. UEID: CJ4VFCDZ1KH5.
FERNANDO I. ALVARADO MUÑOZ DBA AM GROUP: El subcontratista proveerá Servicios Arqueológicos para la Fase 1 del Centro Desarrollo Artístico y Cultural (Antigua Escuela Luis Muñoz Rivera), bajo el Programa de Revitalización de la Ciudad, de los Fondos CDBG-DR. La vigencia del contrato será desde el 12 de junio de 2024 hasta el 31 de agosto de 2024. El monto del contrato no excederá de $12,710.00.
FERNANDO I. ALVARADO MUÑOZ DBA AM GROUP: AMENDMENT A-The Amendment A serve the purpose of extend the contract term from July 11, 2024 until September 30, 2024 and include CDBG-DR mandatory clauses. All other clauses remain unaltered. The subcontractor will provide ARCHAEOLOGICAL STUDIES, for the “Centro Desarrollo Artístico y Cultural (Antigua Escuela Luis Muñoz Rivera)” Project, Phase 1, under the City Revitalization Program of CDBG-DR Funds.
