TEMPLATE TO DOCUMENT EFFORTS
This template provides subrecipients and contractors an easy way to document efforts for hiring and contracting section 3 individuals, section 3 businesses and M/WBE businesses throughout the program lifecycle. Documenting your efforts is a federal compliance requirement that all subrecipients and direct contractors utilizing CDBG-DR funding are responsible for managing and sharing their documented efforts quarterly with PRDOH.
